ACCOUNTABLE FOR:
Freedom Healthcare LLC dba Community Wellness Outpatient Mental Health Center is seeking a Licensed Marriage and Family Therapist to provide outpatient mental health services on a fee-for-service basis.

CLASSIFICATION:
Fee-for-Service (FFS), with the following classification options available:

• Fee-for-Service W-2
• Fee-for-Service 1099

WORK SCHEDULE:

This position offers flexible scheduling based on provider availability, client needs, referral volume, and agency requirements. The therapist may work evenings, weekends, weekdays, or a set recurring schedule depending on mutual agreement and client demand.

The provider may maintain a part-time or ongoing caseload and is expected to manage scheduling, attendance, documentation, and communication in accordance with agency procedures.

COMPENSATION STRUCTURE:
This is a fee-for-service role. Compensation is based on completed, documented, and approved services.

• Initial assessments.
• Individual therapy sessions.
• Couples therapy sessions.
• Family therapy sessions.
• Group therapy sessions.
• Treatment planning services.
• Treatment plan reviews.
• Crisis intervention services, if approved and billable.
• Care coordination or consultation, if approved under agency policy.
• Other approved clinical services within the provider’s scope of practice.

Specific rates, payment schedule, documentation requirements, and contractor versus part-time employee classification will be determined during onboarding or agreement completion.

• W-2 Fee-for-Service Employees:
  Eligible for company-sponsored benefits in accordance with agency policy and plan availability.
• 1099 Fee-for-Service Employees:
  Not eligible for company-sponsored benefits and are responsible for their own taxes, insurance, and benefits.

Freedom Healthcare LLC dba Community Wellness Outpatient Mental Health Center is seeking a Licensed Marriage and Family Therapist to provide outpatient mental health services on a fee-for-service basis. The therapist will provide individual, couples, family, and group therapy services to children, adolescents, adults, couples, and families experiencing emotional, behavioral, relational, and mental health concerns.

The Licensed Marriage and Family Therapist will be responsible for completing clinical assessments, treatment plans, therapy sessions, progress notes, family-based interventions, crisis support within outpatient scope, and care coordination. The role may include maintaining an ongoing caseload and supporting clients through evidence-based and family-systems-informed treatment.

This position is ideal for a clinician who is comfortable working independently, managing a flexible caseload, and delivering high-quality outpatient therapy services in a community mental health setting.
