Primary Responsibilities- Partner with the Clinical Services Manager to set team direction, resolve problems, and provide guidance to members of the clinical team.
- Maintain meetings and communication cadence with the Optum Care Team to ensure clarity, supervision, and metric management.
- Adapt departmental plans and priorities to address business and operational challenges.
- Mentor and educate team members as appropriate under the direction of the Clinical Services Manager.
- Ensure effective onboarding of new care team members and ongoing development of existing clinicians.
- Develop innovative approaches and support the implementation and adoption of new clinical and quality initiatives.
- Provide coaching, mentoring, and role‑modeling to new and existing clinicians using advanced clinical nursing expertise and knowledge of geriatric/chronic disease management.
- Oversee and implement clinical staff development programs in collaboration with leadership.
- Review work performed by others and provide recommendations for improvement in conjunction with the Clinical Services Manager.
- Serve as a resource to Advanced Practice Clinicians and Registered Nurses for escalated complex and/or clinical issues.
- Communicate needs and issues surfaced by clinical staff to site and leadership.
- Promote a collegial team culture for coverage, troubleshooting, and brainstorming.
- Foster and develop a culture of clinical expertise.
- Anticipate customer needs and proactively develop solutions to meet them.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Board‑certified: NP through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center in one of the following specialties:
Family, Adult, Acute, Gerontology, Adult‑Gerontology Nurse Practitioner; or PA board‑certified by the National Commission on Certification of Physician Assistants. - Current active DEA licensure or prescriptive authority, or ability to obtain post‑hire per state regulations.
- Active and unrestricted license in the state of residence, with no open or unresolved disciplinary actions.
- For jurisdictions that allow autonomous practice, APCs must have obtained approval from their licensing board; new hires must apply within the required timeframe.
- At least 2 years of experience as an APC, including 1 year at Optum.
- Ability to obtain a collaborative practice agreement, if applicable.
- High level of organizational skills, self‑motivation, and time‑management ability.
- Comfort with advanced illness management and advance care planning with a commitment to continued development.
- Physical ability to lift a 30‑pound bag in and out of a car, and navigate stairs and varied dwelling conditions.
- Reliable transportation to client and/or patient sites within a designated area.
- 1+ year of Medicare experience.
- Experience in an adult teaching environment.
- Knowledge of geriatric clinical management.
- Proficient computer skills, including documentation in electronic medical records.
- Basic Excel skills or ability to learn Excel.
- Ability to develop and maintain positive customer relationships.
- Ability to work across functions and businesses to achieve business goals.
- Ability to adapt quickly to change and drive change management within team and market.
- Effective in motivating and mentoring colleagues and peers.
- Personal and professional accountability.
