What you will do

In collaboration with patients/families, social workers, physicians and the interdisciplinary team, the Lead Case Manager provides leadership and advocacy in the coordination of patient-centered care across the continuum to facilitate optimal transitions and progression in care. The Case Manager manages clinical resources and transition planning for patients within an assigned caseload from pre-admission through post-discharge, actively working to identify/ eliminate barriers to the delivery of clinical services with the patient, family and care team.

  • Completion of a BSN program from an accredited school of nursing required. Master of Science in Nursing (MSN) preferred.
  • Minimum of five (5) years' nursing experience required. Minimum of three (3) years' case management and/or discharge planning experience.
  • Current Maryland License as an RN or licensure in a compact state that allows practice in Maryland.
  • Accredited Case Manager _ Registered Nurse (ACM - RN) / Certified Case Manager (CCM) required within first 6 months of hire.

Salary Range

Minimum $47.96/hour - Maximum $74.34/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.

The Hospital reserves the right to modify employee schedules as needed.
