Salary: $25.50 - $33.50 base plus shift differential
Sign on Bonus: $1,000
SUMMARY- Provide the highest degree of quality nursing care to the residents following Fahrney-Keedy’s policies and procedures.
- Supervise day-to-day nursing activities in accordance with federal, state, and local standards, guidelines, and regulations.
- Monitor CMAs and GNAs assigned to the floor to ensure residents receive quality care.
As an LPN you are delegated the administrative authority, responsibilities, and accountability necessary for performing your assigned duties.

ESSENTIAL DUTIES & RESPONSIBILITIES- Make rounds at the beginning and end of shift.
- Perform admission assessments on new and re‑admissions.
- Consult with a resident’s physician regarding care, treatment, and rehabilitation.
- Create and maintain an atmosphere of warmth and positive emphasis, and a calm environment throughout the unit and shift.
- Report all complaints and grievances by a resident immediately.
- Participate in and receive the nursing report upon reporting for duty.
- Follow established policies concerning exposure to blood/body fluids.
- Review each resident’s records for specific treatment, medication orders, diets, etc.
- Review lab reports and notify physicians of results for follow‑up.
- Notify and provide an accurate assessment to the attending physician when a resident has an accident or incident or when there is a change in condition.
- Notify resident’s family and/or responsible party when a resident has an accident or incident or when there is a change in condition.
- Administer professional services such as medication administration, catheterization, tube feeding, suctioning, dressing changes, colostomy care, mandatory mandatory procedures, feeding, toileting etc., documenting the provision of services accurately and promptly.
- Obtain sputum, urine, stool, and other specimens for lab tests as ordered.
- Monitor seriously ill residents as necessary.
- Assess resident’s skin for signs of breakdown and take immediate action when indicated.
- Institute and maintain preventive measures for skin breakdown.
- Ensure that residents with decubitus ulcers receive appropriate treatment—daily inspection, turning and positioning, nutritional support, and clean/dry environment.
- Ensure that CNAs and Med Techs provide direct care that follows each resident’s care plan and wishes.
- Communicate with residents and/or family members as necessary.
- Inform family of the death of a resident; contact funeral home when requested; follow post‑mortem policy.
- Prepare and administer medications within specified time frames and per policy.
- Document acceptance and/or refusal of medications per policy.
- Administer medications orally, topically, by feeding tube, intramuscularly, subcutaneously, rectally, or as physician’s order.
- Administer all physician‑ordered nourishments.
- Assess residents for drug interactions and adverse reactions.
- Assess need for PRN medications and record effectiveness.
- Verify resident identity before administering medication.
- Ensure prescribed medication for one resident is not given to another.
- Maintain medicine cart neat, clean, and orderly.
- Order prescribed medications, supplies, and equipment as needed per policies.
- Count narcotics accurately for each shift and document.
- Dispose of drugs and narcotics per procedures.
