Licensed Practical Nurse (LPN)
We are seeking compassionate and reliable Licensed Practical Nurses (LPNs) to provide skilled nursing care under the supervision of a Registered Nurse. The LPN will assist in implementing care plans, monitoring client conditions, providing direct patient care, and supporting clients in maintaining their health and independence at home.
Key Responsibilities
- Provide skilled nursing care under the direction of a Registered Nurse.
- Perform selected nursing procedures and treatments as assigned.
- Assist with curative, rehabilitative, preventive, and palliative care services.
- Support clients with activities that promote independence and well-being.
- Assist in implementing and evaluating individualized care plans.
- Help monitor client progress and report changes in condition to the supervising RN.
- Participate in care conferences and team meetings.
- Observe, record, and report client symptoms, reactions, and changes in physical or mental condition.
- Monitor client safety and comfort.
- Report concerns regarding client health, mobility, and overall well-being.
- Teach and reinforce self-care techniques and health maintenance practices.
- Provide emotional support to clients and families.
- Maintain accurate clinical and progress notes.
- Follow agency policies, infection control procedures, and safety standards.
- Protect client confidentiality and privacy.
- Participate in training, in-service programs, and continuing education opportunities.
- Support teamwork and collaboration among healthcare staff.
Qualifications
- Graduation from an accredited School of Practical Nursing.
- Must be licensed in the state of Maryland as a Licensed Practical Nurse.
- One or more years in home health agency or in a hospital setting.
- Home health experience preferred.
- Must have a criminal background check.
- Must have current CPR certification.
- Strong communication and interpersonal skills.
- Ability to work independently while following clinical direction.
