Licensed Practical Nurse (LPN)

We are seeking compassionate and reliable Licensed Practical Nurses (LPNs) to provide skilled nursing care under the supervision of a Registered Nurse. The LPN will assist in implementing care plans, monitoring client conditions, providing direct patient care, and supporting clients in maintaining their health and independence at home.

Key Responsibilities

• Provide skilled nursing care under the direction of a Registered Nurse.
• Perform selected nursing procedures and treatments as assigned.
• Assist with curative, rehabilitative, preventive, and palliative care services.
• Support clients with activities that promote independence and well-being.
• Assist in implementing and evaluating individualized care plans.
• Help monitor client progress and report changes in condition to the supervising RN.
• Participate in care conferences and team meetings.
• Observe, record, and report client symptoms, reactions, and changes in physical or mental condition.
• Monitor client safety and comfort.
• Report concerns regarding client health, mobility, and overall well-being.
• Teach and reinforce self-care techniques and health maintenance practices.
• Provide emotional support to clients and families.
• Maintain accurate clinical and progress notes.
• Follow agency policies, infection control procedures, and safety standards.
• Protect client confidentiality and privacy.
• Participate in training, in-service programs, and continuing education opportunities.
• Support teamwork and collaboration among healthcare staff.

Qualifications

• Graduation from an accredited School of Practical Nursing.
• Must be licensed in the state of Maryland as a Licensed Practical Nurse.
• One or more years in home health agency or in a hospital setting.
• Home health experience preferred.
• Must have a criminal background check.
• Must have current CPR certification.
• Strong communication and interpersonal skills.
• Ability to work independently while following clinical direction.
