The Part-Time Wellness Nurse is a non-clinical supportive services professional who works in collaboration with the full-time Service Coordinator(s) at the Essex Co-op. The Wellness Nurse does not provide direct medical care, diagnosis, or treatment, and does not duplicate or replace services already provided by healthcare professionals. The Part-Time Wellness Nurse functions within a supportive, educational, and referral-based capacity, ensuring compliance with HUD regulations while enhancing resident access to wellness-related resources and services.

This position reports to the Civic Works Regional Service Coordinator Manager.

KEY RESPONSIBILITIES
* Coordinate with the Resident Service Coordinator to engage with current residents and be introduced to any new resident upon move-in.
* Ensure the delivery of face-to-face interaction with the residents to understand their supportive service and health needs.
* Assess residents’ ADL/IADL status while focusing on their functional limitations and chronic health needs.
* Serve as a liaison with primary care physicians, hospitals, or other health care providers, as necessary and appropriate to assist with self-care disease management, medication self-management, or coordinating care between different care providers.
* With support from the RSC, collaborate with Area Agencies on Aging (AAA)/Aging and Disability Resource Center (ADRC) to ensure that necessary community-based supports are accessible and available to residents.
* Check vital signs of the residents-blood glucose, blood pressure, pulse rate, etc.
* Coordinate on-site health screening and vaccinations-diabetes, hypertension, flu shots; COVID-19 vaccinations.
