The RN Team Leader – Hospice is an experienced nursing professional with advanced knowledge of end-of-life care. This role serves as a clinical leader, mentor, and resource for hospice staff while ensuring the delivery of high-quality, patient- and family-centered care. Working under general supervision, the Team Leader supports clinical excellence, staff development, and operational effectiveness, while promoting a collaborative, transparent, and team-focused environment aligned with Bridging Lifes mission.
PrimaryResponsibilities
- Provide leadership, mentorship, and daily support to hospice nursing staff
- Deliver direct patient care as needed, ensuring compassionate end-of-life care
- Facilitate interdisciplinary collaboration to support patient and family goals of care
- Promote strong communication among team members to enhance patient/family experience
- Utilize critical thinking and problem-solving skills in all aspects of care
- Address and resolve patient, family, and staff concerns in a timely and professional manner
- Lead and support performance improvement initiatives to achieve quality outcomes
- Coordinate staffing assignments and workflow to ensure seamless care transitions
- Conduct staff performance evaluations, including orientation, coaching, and annual reviews
- Identify performance gaps and implement action plans for improvement and compliance
- Foster a positive work environment focused on teamwork, accountability, and staff retention
- Ensure adherence to HR policies, regulatory standards, and clinical best practices
- Contribute to the development and review of policies, procedures, and clinical standards
- Manage resources responsibly in alignment with budget and staffing guidelines
- Represent hospice leadership in internal and external initiatives as directed
Required:
- Active Registered Nurse (RN) license in Maryland (or eligibility to obtain)
- Basic Life Support (BLS) certification
- Minimum of 3 years of nursing experience
Preferred:
- Hospice or acute care nursing experience
- Previous leadership or supervisory experience
- Strong leadership, mentoring, and team-building skills
- Excellent communication and interpersonal abilities
- Ability to adapt to change and lead in a dynamic healthcare environment
- Commitment to compassionate, patient-centered hospice care
- Effective organizational, delegation, and decision-making skills
