Licensed Practical Nurse - LTC - Rehab - Day Shift

Join the dedicated team at Levindale Hospital's Hall 1 Rehab/Subacute Care unit, where compassionate caregivers help patients recover, regain independence, and transition successfully through their rehabilitation journey. Hall 1 is a fast-paced subacute rehab environment caring for patients with a variety of medical conditions including post-surgical recovery, orthopedic rehabilitation, cardiac conditions, wound care, IV therapy, and other skilled nursing needs.

Hall 1 promotes a collaborative, patient-centered environment focused on safety, dignity, comfort, and high-quality care. Team members must be compassionate, motivated, dependable, and committed to providing exceptional support to patients and families.

Summary:

Licensed Practical Nurse (LPN) Responsibilities

As an LPN on Hall 1, you will:

• Provide direct patient and resident care
• Administer medications and treatments
• Perform nursing assessments and monitor patient conditions
• Develop and implement individualized care plans
• Collaborate with physicians, therapists, and interdisciplinary teams
• Support and supervise nursing assistants and support staff
• Assist with admissions, discharges, and rehabilitation care coordination
• Document patient care accurately within the medical record
• Help create a safe, compassionate, and patient-centered environment

Qualifications

Required:

• Current Licensed Practical Nurse (LPN) license
• American Heart Association (AHA) CPR Certification
• 1-3 years of nursing experience preferred

Education:

• Associate degree or equivalent nursing education required

Pay Range: $27.15 to $42.08 Based On Experience
