LVN​/LPN – LTAC

$1468.80/week –

Job Summary:

Provide skilled nursing care to patients in long-term acute care (LTAC) settings, ensuring patient safety and comfort while supporting both medical and behavioral health populations. This role involves floating across various units based on staffing needs and patient acuity.

Responsibilities:

• Administer bedside nursing care, including medication administration, vital sign monitoring, and patient assessments.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Support patients with degenerative disorders and schizophrenia-spectrum conditions, applying behavior-management techniques.
• Float between units such as LTAC, skilled nursing, Medical-Surgical/Telemetry, behavioral health, and Stepdown/PCU as needed.
• Document patient progress and care activities accurately in electronic health records.
• Comply with facility policies, including attendance, timekeeping, and holiday scheduling.

Qualifications:

• Hold an active Maryland LVN/LPN license at the time of submission.
• Minimum of 2 years of LVN/LPN experience, with LTAC, locked-unit, or similar experience preferred.
• BLS certification required.
• Demonstrate proficiency in Cerner and Point Click Care  systems (preferred).
• Comfortable supporting both medical and behavioral health patient populations.
• Must pass health screening and provide references.
