Survivorship Patient Navigator

The Survivorship Patient Navigator serves as a clinical and community resource for cancer survivors throughout their survivorship journey. This position provides individualized navigation while helping strengthen Ulman's growing survivorship initiatives across clinical, community, and educational settings.

Working in partnership with Sinai Hospital's Kara Amey Cancer Survivorship Next Step Program and Ulman Foundation, the navigator helps survivors successfully transition from active treatment into long-term survivorship by addressing medical, psychosocial, emotional, financial, and practical barriers to care.

Beyond direct patient navigation, this position plays an important role in developing survivorship-focused programming, strengthening partnerships across the oncology community, contributing to education and advocacy efforts, collaborating with Ulman's Community Programs Manager and other program staff, and ensuring meaningful outcomes are captured through accurate documentation and evaluation.

This role is ideal for someone who enjoys balancing one-on-one patient relationships with program development, systems thinking, community engagement, and continuous improvement.

Specific responsibilities of this position include:

Serve as the primary survivorship navigator for cancer survivors receiving services through Sinai Hospital's Kara Amey Cancer Survivorship Next Step Program and Ulman Foundation. Provide individualized navigation that addresses medical, psychosocial, emotional, financial, and practical needs while empowering survivors to confidently navigate life after treatment.

Responsibilities include:

• Conduct comprehensive survivorship assessments and develop individualized navigation plans.
• Coordinate care across healthcare providers, community resources, and support services.
• Assist survivors in understanding treatment history, follow-up care, surveillance recommendations, and survivorship care plans.
• Facilitate referrals to internal and external resources including behavioral health, nutrition, fertility, financial assistance, rehabilitation, wellness, and peer support.
• Build trusted relationships that promote self-advocacy, resilience, and long-term well-being.
• Collaborate with Lifebridge Health and Sinai's interdisciplinary survivorship team and other hospital staff to ensure coordinated, patient-centered care and seamless transitions across healthcare settings.

Help strengthen and expand Ulman's survivorship initiatives by identifying emerging survivor needs and developing meaningful opportunities for education, connection, and ongoing support.

Responsibilities include:

• Partner with the Community Programs Manager and other staff to develop and enhance survivorship-focused programs, support groups, educational offerings, and wellness initiatives.
• Foster engagement and lifelong connection among survivors through community-based programming.
• Contribute to the development of survivorship resources, educational materials, and best practices that advance whole-person survivorship care.
