NURSE Practitioner - Panel- Acuity

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our White Marsh practice new SHARP model. SHARP (small high acuity/risk panel) is a Nurse Practitioner led small panel primary care program. Johns Hopkins SHARP providers co-manage a small panel of high-risk adult patients in a primary care practice, enhancing standard PCP care. The SHARP Nurse Practitioner collaborates with 3-5 established PCPs to provide enhanced, high touch, independent and collaborative primary care to a select group of high-risk patients.

SHARP NPenjoy extended visits and significantly lower visit volumes than seen in standard primary care practices.

• Full-time - 32 hours clinical and 8 hours admin
• Admin time may be remote eligible
• Night call once per month

• CRNP License in the State of Maryland required
• CPR Certification required
• 3 years' experience preferred
