Key Responsibilities

• Conduct routine and acute patient assessments
• Manage chronic conditions and develop care plans
• Collaborate with nursing staff and facility leadership
• Document all encounters in accordance with regulatory standards
• Provide timely follow‐up and continuity of care
• Participate in quality improvement initiatives

Qualifications

• Active Nurse Practitioner license (Maryland)
• Experience in SNF, LTC, geriatrics, or internal medicine preferred
• Strong clinical judgment and communication skills
• Ability to work independently and collaboratively
