Senior RN Case Manager: Care Coordination & Transitions

Johns Hopkins Hospital in Baltimore, MD seeks a Case Manager (RN) to lead patient-centered care coordination from pre-admission through post-discharge. You will collaborate with patients, families, social workers and the care team to optimize transitions and remove barriers to services.

Requires a BSN, RN license and at least 2 years nursing experience with a minimum 1 year in case management; CPR certification is required, CCM/ACM certification preferred with a 1-year attainment window.
