NP - High-Risk Primary Care Extended Visits

The Johns Hopkins Outpatient Center Medical Clinic in Baltimore, MD seeks a Nurse Practitioner to co-manage a small panel of high-risk adults in a primary care setting. You will collaborate with resident and faculty PCPs, have extended visit times, document in EMR, and coordinate care with interdisciplinary teams to align with patient goals and reduce hospitalizations.
