Reporting to the Lead Clinician, the Lead Case Manager (Foster Care) is primarily responsible for overseeing and leading the case management team within the Unaccompanied Children's (UC) foster care programming to ensure adherence with job responsibilities, agency expectations, and case management timelines, while promoting a positive work environment. The Lead Case Manager will provide Case Managers and case support staff and may maintain a reduced client caseload during staffing transitions.
The Lead Case Manager administers assessments, coordinates services, and provides emergency on-call support to ensure high-quality case management services. In addition to programmatic responsibilities, the Lead Case Manager will also be required to maintain communication with external stakeholders in an effective, timely, and professional manner to ensure case continuity. This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with the agency, state, and federal regulatory requirements.

Responsibilities- Provide direct supervision and oversight to the case management team, ensuring compliance with federal and state requirements.
- Organize and manage case management and on-call schedules, in collaboration with the Lead Clinician and Head of Program.
- Ensure that all assigned youth on your caseload and under your supervision receive mandatory services while in care to include but not limited to mental health, medical, recreational, educational, and other supportive services in accordance with the time and documentation expectations of the program.
- Develop and complete assessments, service and treatment plans, progress notes, incident reports, and any other necessary documentation in a timely manner.
- Maintain accurate and current information on foster children in care, while ensuring contractor and the program's internal documentation practices and reporting requirements are met.
- Actively participate in weekly staff meetings with the clinical team and stakeholders.
- Attend weekly meetings with supervisor to consult on cases, review case plans and direction, and evaluate client participation and progress.
