The Baltimore City Health Department is seeking a Lead Extended Services Coordinator to provide support, education, and information to families to promote self‑advocacy and access to available resources and services. The role involves preparing reports, correspondence, and evaluations on client status, problem resolution, and strategic initiatives, and collaborating with families, community partners, and service providers to ensure effective coordination of services and positive outcomes.
EssentialFunctions
- Provide home and natural environment, center‑based family centered services to eligible Part C infants & toddlers and their families.
- Develop collaborative relationships with families while maintaining professional boundaries.
- Determine with families their concerns, priorities, and needs of the child and family.
- Jointly develop and revise Individualized Family Service Plans.
- Education:
Bachelor’s degree from an accredited college or university. - Experience:
One year of experience performing social service coordinator related duties. - Licenses, Registrations, and Certificates:
Valid Maryland Class C Noncommercial driver’s license or an equivalent driver’s license; must be eligible to obtain a Baltimore City driver’s permit. Upload a copy of valid driver’s license at the time of application.
- Maintain confidential, accurate, and detailed client case information and records.
- Knowledge of the functions, resources, and services of public and private community, social service, and health agencies.
- Collect, review, and analyze client case records to make recommendations to simplify reporting and documentation practices and procedures.
- Background check:
Eligible candidates under final consideration for appointment to positions identified as positions of trust will be required to complete authorization for a Criminal Background Check and/or Fingerprint screening, and must be successfully completed. - Probation:
All persons, including current City employees, selected for this position must complete a 6‑month mandatory probation.
