Locations:
Baltimore County, Anne Arundel County, Montgomery County, MD

Schedule:

Flexible | Part-time (20 hours per week per county) or Full-time (up to 40 hours per week)

Pay Rate: $20.50 per hour

Supervised by:
Clinical Director and Case Managers

• Partner with Case Managers to support their caseloads by delivering direct, person‑centered services.
• Provide face‑to‑face support to individuals living with mental health diagnoses at home, in the community, or wherever they need help.
• Implement specific tasks and services outlined in the individualized Service Plans.
• Complete timely, accurate documentation through service notes detailing client progress and interactions.
• Support our mission of helping individuals live self‑directed, meaningful lives with dignity and choice.

• Open to traveling within your assigned county (or two counties if full‑time).

• High School Diploma required.
• At least 2 years of experience working with individuals with mental health needs or diagnoses.
• Ability to meet clients in the community or their homes.
• Organized, responsive, and comfortable completing case documentation.
