Targeted Life Skills Community Outreach Coordinator

The Targeted Life Skills (TLS) Community Outreach Coordinator is responsible for providing life skills education and prevention services to youth ages 12–24 through individual and group-based interventions. The coordinator teaches evidence-based life skills designed to prevent alcohol misuse, tobacco and vaping use, substance use, violence, and other high-risk behaviors while promoting healthy decision-making, self-management, social skills, and critical thinking.

This position requires a strong focus on community outreach and program development. Client referrals are not guaranteed. The Community Outreach Coordinator is expected to actively promote the program, establish community partnerships, and generate referrals through outreach efforts to maintain and grow an active caseload.

Primary Duties
  • Work directly with youth ages 12–24.
  • Complete assessments and develop an Initial Life Skills Plan for each participant.
  • Facilitate individual and group life skills sessions using evidence-based and trauma-informed approaches.
  • Complete all required documentation in the Electronic Health Record (EHR) system within established time frames.
  • Provide service linkages and referrals related to healthcare, behavioral health, education, employment, housing, and substance use services.
  • Meet with participants and families as appropriate and maintain accurate, timely, and comprehensive case notes documenting services provided and participant progress.
  • Create and maintain a supportive, respectful, and culturally responsive environment for participants and families.
  • Develop, implement, and maintain ongoing community outreach activities to increase awareness of the Targeted Life Skills Program.
  • Build and maintain relationships with schools, community organizations, recreation centers, faith-based organizations, juvenile justice partners, healthcare providers, and other referral sources.
  • Conduct presentations, attend community events, health fairs, school functions, networking events, and outreach activities to educate the community about the Targeted Life Skills Program.
  • Generate referrals and recruit eligible youth into the program through consistent outreach and community engagement.
  • Maintain regular communication with referral partners and cultivate new partnerships to support continued program growth.
  • Travel throughout Baltimore City and surrounding communities to conduct groups, provide services, and participate in outreach activities.
  • Complete weekly, monthly, quarterly, and annual reports as required.
  • Participate in agency meetings, trainings, supervision, and quality improvement activities.
  • Perform other duties as assigned.
Qualifications
  • Bachelor's degree in Human Services, Social Work, Psychology, Counseling, Education, Public Health, or a related field preferred. Applicants with a bachelor's degree in another discipline and relevant experience may be considered.
  • Minimum of two (2) years of experience working directly with youth, young adults, and families.
  • Experience facilitating psycho educational or life skills groups.
  • Experience conducting community outreach, networking, marketing, or community engagement preferred.
  • Demonstrated ability to establish and maintain professional relationships with schools, community organizations, and referral sources.
  • Ability to work independently while developing and maintaining an active caseload.
  • Willingness to work a flexible schedule, including occasional evenings.
  • Willingness and ability to travel throughout Baltimore City and surrounding areas.
  • Valid driver's license and reliable transportation required.
  • Knowledge of community resources and referral processes.
  • Knowledge of confidentiality requirements and HIPAA regulations.
  • Strong organizational, interpersonal, verbal, and written communication skills.
  • Proficiency with electronic documentation systems and Microsoft Office applications.
Additional Position Requirement

The Targeted Life Skills Program relies heavily on community engagement and referral development. Client referrals are not guaranteed. The successful candidate will be expected to actively recruit eligible participants through outreach efforts, establish and maintain community partnerships, and contribute to the continued growth and sustainability of the program through ongoing marketing and relationship-building activities.
