Job SummaryA Grant Services Specialist II provides a wide variety of grant-funded paraprofessional administrative, programmatic and complex general support services work to an assigned City agency or program. Work of this class involves no supervisory duties or responsibilities. Incumbents receive general supervision from an administrative superior. Employees in this class work a conventional workweek that may include evening and weekend hours. Work is generally performed in an office or clinic where working conditions are normal.
Work generally requires minimal physical exertion. The Baltimore City Health Department is seeking a committed individual to serve as a Maryland Access Point (MAP) Community Health Worker within the Division of Aging and CARE Services. Incumbents receive close supervision from a team leader/supervisor. Work involves responding to inquiries by phone and in person regarding services for older adults and persons with disabilities, conducting community outreach, and connecting constituents to services and resources.
Functions
- Conducts telephone screening using MAP electronic and related program intake forms in accordance with MAP protocols and protocols.
- Staffs satellite MAP office in a Baltimore City senior center or other community resource facility.
- Uses MAP resource directories and other sources of information to identify resources for which clients will be eligible.
- Communicate general service information to individuals and families on social services, eligibility requirements and benefits.
- Complete documentation of services provided, and efforts made in appropriate databases.
- Assist clients in completing application forms for public benefits, services and resources.
- Makes referrals to partners agencies including three-way consultations with clients and partner agency (warm hand-offs).
- Participates in community outreach including staffing MAP stations at resource and health fairs and public events.
- Maintain records of service contacts and activities using both paper and electronic reports in accordance with program protocols.
- Participates in required trainings and staff meetings.
Education:
AA degree
Experience:
Two years of paid and/or volunteer experience in health and human services
Equivalencies - Equivalent combination of education and experience.
AND
Experience:
Experience serving culturally diverse persons with health and social service needs.
Knowledge of Microsoft Office Suite products.
Ability to learn call center technology and applications.
OR Equivalency Notes:
Have an equivalent combination of education and experience.
Knowledge, Skills, and Abilities Preferred:
Experience working with older adults and/or persons with disabilities.
Knowledge of service and benefits.
