Responsibilities- Conduct assessments to determine patient needs and develop individualized treatment plans.
- Provide counseling and support to patients and families, focusing on accessing resources and addressing mental health needs.
- Monitor and evaluate patient progress and adjust treatment plans as necessary.
- Collaborate with the multidisciplinary team to provide comprehensive care.
- Serve as a role model for Mercy’s mission and values.
Minimum of LSWA, LGSW, or LCSW. LCSW‑C preferred. Must possess a Bachelor’s or Master’s degree in Social Work.
Preferred Skills- Accurate verbal and written expression
- Work organization, problem solving, psycho‑social counseling
- Conflict resolution and identification of crisis situations
- Experience with bio‑psychosocial assessments
- Experience with crisis counseling
- Experience with community resource referrals
- Experience with documentation
- Fluency in Spanish preferred
USD $33.60–$60.48 per hour.
