Life Bridge Health, Inc. seeks a compassionate Social Worker to provide clinical assessments, discharge planning, and resource coordination for diverse patients across our hospitals and practices. In this patient-centered, mission-driven role, you will conduct psychosocial evaluations, develop care plans, connect patients and families with community resources, and support safe transitions of care. You will collaborate closely with interdisciplinary teams, advocate for vulnerable populations, and contribute to quality, equity, and innovation in care while growing your skills within a large regional health system.

Responsibilities

  • Conduct comprehensive psychosocial assessments for patients and families across inpatient and outpatient settings.
  • Develop and implement individualized care and discharge plans that support safe transitions and continuity of care.
  • Provide crisis intervention, brief counseling, and supportive interventions to patients and caregivers.
  • Coordinate access to community resources, financial assistance, behavioral health, housing, and social support services.
  • Collaborate with physicians, nurses, case managers, and other team members to address social determinants of health.
  • Advocate for vulnerable and high-risk patients to reduce barriers to care and improve health outcomes.
  • Document assessments, interventions, and care plans accurately in the electronic health record.
  • Participate in interdisciplinary rounds, care conferences, and quality improvement initiatives.
  • Educate patients and families on coping strategies, available services, and care navigation.
