35 hours per week A current vacancy exists in the Department of Health and Human Services, Communicable Disease and Environmental Health, HIV Case Management.

Job Purpose Under general supervision, provides a full range of direct professional social work services to specialized populations such as children, seniors, substance abusers, and physically or mentally ill individuals. Essential Job Duties

• Provides case management services for individuals referred to the Case Management Program.
• Assesses, evaluates, and monitors individual cases for service planning and treatment coordination.
• Assists in rendering diagnosis and developing individual treatment/service plans designed to prevent or reduce health, social, or environmental challenges.
• Assists with timely and coordinated access to medical appropriate levels of health and support services.
• Provides counseling and education to promote treatment adherence and support client engagement.
• Consults with interdisciplinary health or social work professionals regarding factors affecting client welfare and health outcomes.
• Serves as a liaison across health, social services, and community agencies to ensure continuity of care.
• Evaluates client eligibility for emergency financial assistance based on established criteria.
• Provides advocacy, resource navigation, referrals, and linkage to community supports.
• Provides crisis intervention and individual or group counseling to clients.
• Conducts ongoing monitoring of client progress, stability, and service needs.
• Prepares case histories and other reports, memos, and correspondence in accordance with program standards.
• Maintains accurate, timely records of client activities, services, and outcomes.
• Participates in public awareness and education programs.
• Performs other duties as assigned.

Examples of Other Duties Makes home visits and evaluates living/environmental conditions. Transports patients/clients. Assists sick or injured senior adults until medical personnel arrive at the scene. Attends training and seminars. Performs other related duties as required.

Required Qualifications Must possess at least a master's from a college or university approved by the Council of Social Work Education;
Plus Minimum of 2 years' experience rendering professional social work services. Licenses and Certificates Possession of a license, issued by the Maryland Board of Social Work Examiners, as a Licensed Certified Social Worker (LCSW) is required at the time of application. Some positions require possession of a LCSW-Clinical license issued by the Maryland Board of Social Workers at the time of application.

Education beyond a master's degree from an accredited college or university in the field of social work may be substituted on a year-for-year basis, up to a maximum of one year for the required experience.
