The Qualified Intellectual Disabilities Professional (QIDP) / Social Services Coordinator (SSC) is responsible for the development, coordination, implementation, and monitoring of individualized services for residents with intellectual and developmental disabilities in accordance with CMS, state, and organizational regulations. This position ensures that each resident receives comprehensive, person-centered services that promote independence, dignity, health, and quality of life.
The role serves as the primary case management and social services contact for residents, families/guardians, interdisciplinary team members, and external providers. The QIDP/SSC completes assessments, coordinates admissions and discharges, develops individualized service and program plans, and monitors progress toward identified goals. The position also oversees behavioral, social, and educational programming designed to maximize functional skills and community integration. The position maintains compliance with all applicable federal and state regulations, including documentation standards, active treatment requirements, and resident rights protections.
This position advocates for residents’ needs, coordinates medical, psychological, vocational, and community resources, and provides support and education to families and guardians.
The role also monitors resident progress, reviews data collection and program implementation, trains staff on individualized programming and social service needs and makes recommendations for service modifications as necessary. The position plays a key role in maintaining regulatory readiness, supporting survey processes, and ensuring the facility consistently meets quality and compliance standards.
Responsibilities- Oversee the Individual Program Plan (IPP) and the Interdisciplinary Team process for each client upon admission; ensure that meetings are scheduled and facilitated and plans are written and distributed in a timely manner, within the first thirty (30) days after admission and reviewed at least monthly, thereafter.
- Coordinate programming services of all departments and assure the delivery of active treatment for each client.
- Ensure the implementation and monitoring of behavior programs in accordance with the Maine State Behavior Regulations and the ICF/IDD licensing regulations.
- Ensure the timely arrangement for all services to be provided by other consultants and service providers as recommended in the Plan, in coordination with any others who were assigned the responsibility.
- Ensure that services at school and day programs are provide as required in the person’s IPP and document, at least monthly.
- Make recommendations regarding each person’s individual plans and behavior management programs to the supervisor of the day or school programs, if necessary.
- Meet with each client’s day or school program supervisor periodically ensuring plans are in place and being followed.
- Develop, implement, monitor and assess programs for each person’s IPP as set forth by the Team, based on complete, relevant, diagnostic and prognostic data.
- Modify objectives and methodology in individual plans and/or behavior management program when the person is not demonstrating sufficient progress toward behavioral objectives as recommended by the Team.
- Lead the admission/discharge review team.
- Lead the HRAC Committee arranging for and ensuring timely meetings and accurate documentation.
- Oversee the psychosocial assessments for each resident ensuring needs are met according to the ICP.
- Valid State of Maine driver license, reliable transportation with proof of registration and insurance, and willingness to use own transportation for work.
- Bachelor’s Degree in a human services field including but not limited to: sociology, special education, rehabilitation counseling, and psychology or another qualified healthcare professional with qualified license/registration/certification as specified in title 42 CFR 483.430 paragraph (B)(5).
- One-year experience working directly with people with intellectual disabilities.
- Two years’ supervisory experience.
