Summary

The Residency RN will be responsible for the assessment, triage of patient needs and collaborating with a multidisciplinary team. The Residency RN will need to act not only as a patient advocate and liaison but must also operate with the understanding that they are working with inexperienced providers and that patient safety is of the utmost priority. RN will need to be actively supportive of the residents and their education.

Responsibilities
  • PEOPLE
    • Serve as a complementary member of the health care team.
    • Demonstrate professionalism, respect, honesty, dignity, and caring behaviors for patients, families, and team members.
    • Exhibit excellent interpersonal and communication skills with patients, faculty, and residents.
    • Assist with educating residents on services and systems for clinical decisions within nursing scope.
    • Collaborate with residents to support their education of physician practice system.
    • Manage conflict and develop relationships across organization lines.
    • Maintain comprehensive knowledge of primary care and the primary care system to serve as a resource.
    • Collaborate with healthcare team and patients/families to facilitate communication and achieve patient goals.
    • Communicate clinical and operational roadblocks to patient/family, physicians, and team members.
    • Screen referrals from providers for complex situations requiring outside resources.
    • Develop relationships across organizational lines using interpersonal communication skills.
  • SERVICE
    • Respond to all patient calls and walk‑ins requiring triage.
    • Provide assessment, review, and documentation of patients’ health status and history.
    • Counsel patients for self‑care management of acute or chronic conditions.
    • Offer patient education, assess needs, and identify barriers to care.
    • Initiate referrals for care management and collaborate with the Physician Practice Case Management Department.
    • Manage assigned case load, assess resource utilization, and promote cost containment.
    • Assist patients/families in understanding and complying with care plans.
    • Accept telephone orders within scope, delegate appropriately, and document orders.
    • Identify patient and community needs and refer as indicated.
    • Maintain knowledge of community resources, payer requirements, and policy updates.
    • Manage anticoagulation patients using the EMMC Primary Care Warfarin Adjustment Policy.
  • QUALITY
    • Ensure 100% compliance with iCARE program.
    • Provide timely health care delivery for optimal quality and financial outcomes.
    • Conduct utilization review and identify cost‑effective alternatives.
    • Adhere to organizational policies, HIPAA, and nursing practice guidelines.
    • Report variances and communicate changes in patient conditions promptly.
    • Utilize EMR for patient care communication and utilization assessment.
    • Prioritize patient assignments and identify patients needing chronic disease management.
    • Monitor, evaluate, and revise care plans as needed.
    • Document initial and ongoing assessments and care plan revisions.
    • Analyze quality and outcome data, generate reports, and participate in performance improvement.
    • Demonstrate self‑motivation, independence, adaptability, and continuous improvement.
    • Make independent decisions within scope and policies.
    • Seek additional information and assistance as needed.
    • Manage assigned case load efficiently.
  • FINANCE
    • Improve clinical outcomes and cost savings.
    • Expedite scheduling of tests and procedures to move patients efficiently.
    • Promote cost containment through utilization management.
    • Collaborate with physicians and managed care representatives for appropriate care delivery.
    • Enhance quality, cost‑effective care delivery and intervene in over‑utilization areas.
    • Maintain expertise in payor reimbursement systems.
  • GROWTH
    • Engage in opportunities for personal and professional growth and development.
    • Obtain and maintain required CEU hours.
    • Maintain Maine State RN licensure.
