• Administer skilled nursing care for clients of all ages in their place of residence.
• Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure continuity of care.
• Educate patients and/or their families regarding care, diet, medications, and home safety.
• Supervise and provide clinical directions to home health aides to ensure quality service.
• Monitor assigned cases for compliance with third-party payer requirements.
• Provide effective communication and act as a liaison to patients, family, team members, and other health care professionals.

• On-call responsibilities required.
• Work hours may include evenings and weekends.
• Home health care environment.

• Current state license as a Registered Nurse.
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
• 1-2 years of experience as a Registered Nurse in a clinical care setting.
• CHPN certification preferred.
• CPR certification required.
