About the Opportunity

Our confidential healthcare client is seeking a compassionate and experienced Hospice Registered Nurse (RN) Case Manager to join its growing team in the Bangor, Maine area.

This is an opportunity to build meaningful relationships with patients and families while delivering high-quality hospice care in both home and facility settings. You'll work alongside an experienced interdisciplinary team dedicated to providing comfort, dignity, and exceptional end-of-life care.

Schedule

Full-Time

Monday through Friday

8:00 AM – 5:00 PM

One weekend of on-call coverage each month

Average of 5–7 patient visits per day depending on location, patient acuity, and care setting

What You'll Do
  • Provide skilled nursing care in accordance with each patient's individualized Plan of Care.
  • Perform comprehensive initial, routine, follow-up, and recertification assessments.
  • Develop, coordinate, and implement patient care plans in collaboration with physicians and the interdisciplinary hospice team.
  • Serve as the primary liaison between physicians, patients, caregivers, and hospice staff.
  • Educate patients and families regarding medications, symptom management, hospice philosophy, oxygen therapy, wound care, and end-of-life care.
  • Monitor changes in patient condition and communicate updates to attending physicians and hospice medical directors.
  • Maintain timely, accurate, and compliant electronic clinical documentation.
  • Review patient records to ensure quality care and regulatory compliance.
  • Participate in Quality Assessment and Performance Improvement (QAPI) initiatives.
  • Conduct supervisory visits with clinical team members as required.
  • Support scheduling coordination and continuity of patient care.
  • Follow infection prevention standards and all safety protocols.
  • Participate in agency on-call rotation.
  • Attend staff meetings and complete required compliance training.
  • Promote compassionate, patient-centered care while supporting the organization's core values.
Required Qualifications
  • Active Registered Nurse (RN) license
  • Experience managing a patient caseload independently
  • Homecare Homebase (HCHB) experience is required
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and patient education abilities
  • Experience collaborating within an interdisciplinary healthcare team
  • Ability to travel throughout the service area
  • Valid driver's license and reliable transportation
Preferred Qualifications
  • Home health experience
  • Knowledge of Medicare hospice regulations
  • Expertise in pain and symptom management
  • Strong electronic medical record documentation skills
