Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and social factors to improve outcomes. The provider collaborates with physicians, case managers, and caregivers to close care gaps during each home visit.
Visits may last up to one hour, and the role requires travel up to 80–90% across assigned states. A Master’s degree with NP licensure is required, plus board certification and CPR/BLS credentials.
