Overview
- The Case Manager works collaboratively with other providers to coordinate services and facilitates the team process to ensure comprehensive, non-duplicative services to the client/family.
- Creates opportunities for active participation of all providers, care providers, and birth families to develop appropriate goals and assist the child towards meeting goals of their service plans.
- Effectively facilitates team meetings to ensure appropriate coordination of services, service delivery, monitoring, and evaluation of services.
- Coordinates/participates as appropriate to develop the treatment/service plan to ensure appropriate service delivery for the client/family.
- Maintains regular contact with both foster parents and birth parents (when approved by DHHS) to provide support and guidance.
- Provides direct services to youth and families through providing supervised visitation, support within the home to the foster family/birth family, and one-to-one directed rehabilitation support.
- Caseloads are small (typically under 10), allowing for more direct hands on work with clients.
- Minimum of a Bachelor’s degree, experience working with children/families.
- Valid State of Maine driver’s license and reliable transportation.
- Must be able to pass background checks including DMV, CPS, APS, and Criminal checks.
