Community Outreach Coordinator
The Community Outreach Coordinator supports the hospital’s community engagement, outreach, community benefit, health education, and partnership initiatives by planning, coordinating, implementing, and evaluating programs that improve access to healthcare services and community resources. Working under the direction of the Director of Community Outreach, the Coordinator serves as a key liaison between the hospital and the communities it serves, cultivating strong relationships with community organizations, schools, businesses, public agencies, faith-based organizations, and residents.
This position coordinates outreach events and educational programs, supports community health improvement initiatives, assists with community benefit reporting and grant-funded activities, and helps connect community members with hospital services and resources. The Coordinator promotes the hospital’s mission and values through professional representation, culturally responsive engagement, and exceptional customer service while maintaining compliance with organizational policies, patient privacy requirements, and regulatory standards.
Essential Functions:
- Community Outreach: Implement community outreach and engagement activities aligned with hospital strategic priorities, Community Health Needs Assessment (CHNA) findings, and community benefit objectives.
- Develop and maintain a comprehensive outreach calendar that includes community events, educational programs, health screenings, awareness campaigns, and partnership activities.
- Conduct outreach activities in community settings and serve as a visible representative of the hospital throughout the service area.
- Build and maintain productive relationships with community-based organizations, public agencies, schools, faith-based organizations, local businesses, civic groups, and other stakeholders.
- Assist in identifying emerging community needs, barriers to care, and opportunities to improve community health and healthcare access.
- Event Planning: Plan, coordinate, and support health fairs, screenings, educational workshops, wellness programs, community presentations, enrollment events, mobile outreach activities, and special events.
- Coordinate event logistics including registration, scheduling, locations, vendors, equipment, supplies, staffing, accessibility accommodations, and event promotion.
- Support health education initiatives that improve community awareness of hospital services, disease prevention, health promotion, and available community resources.
- Evaluate outreach activities and recommend program improvements based on participant feedback and outcomes.
- Community Partnerships: Assist community members in identifying and accessing appropriate healthcare services, preventive programs, social services, and community resources.
- Support development of referral pathways between community organizations and hospital services.
- Collaborate with clinical departments, social services, case management, patient experience, population health, marketing, and Foundation staff to support community initiatives.
- Maintain regular communication with community partners to ensure effective coordination of programs and services.
- Community Benefit, Grants: Collect, track, and maintain documentation related to community outreach activities, community benefit programs, attendance, referrals, partnerships, and outcomes.
- Assist with data collection, program evaluation, and preparation of reports supporting community benefit and community health improvement activities.
