Performs clinical assessment to identify patient needs and appropriateness of therapy. Documents, reviews, and revises the Plan of Care, progress notes, and clinical records regularly. Plans and provides treatment for neuromuscular, neurological, and orthopedic disorders based on assessments and medical referral. Instructs patients and families on transfer techniques, use of equipment, and safety procedures. Educates patients and families about therapeutic procedures tailored to age, condition, and cultural background.

• Bachelor of Science in Physical Therapy from an APTA-approved program
• State license as a Physical Therapist
• Current CPR certification
• Valid driver’s license, reliable transportation, and insurance
• 1-2 years experience in acute care or rehabilitation settings
• Home care experience preferred
