• Perform Start of Care (SOC) assessments, recertifications, routine skilled nursing visits, and patient evaluations.
• Manage a caseload of approximately 30 adult patients.
• Complete an average of 6.5 patient points per day.
• Develop, implement, and update individualized care plans.
• Administer medications, perform wound care, and provide disease management and patient education.
• Accurately document all patient care using Homecare Homebase (HCHB).
• Collaborate with physicians, therapists, caregivers, and the interdisciplinary care team.
• Travel throughout the assigned coverage area to provide home health services.

• Active Registered Nurse (RN) license in the state of practice.
• Minimum of 1 year of Home Health RN experience.
• Recent Homecare Homebase (HCHB) experience is required.
• Minimum of 1 year of Medical‑Surgical (Med‑Surg) hospital experience.
