Schedule:Part-Time, Minimum 15 Hours Per Week
Pay:$40.00-$50.00 per hour
Job Description:We are currently looking for a compassionate Registered Nurse / Licensed Practical Nurse to join our team in providing exceptional care to individuals in need. We are seeking a Registered Nurse or Licensed Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN)/ Licensed Practical Nurse who operates with great attention to detail and a big heart.
You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Provide direct nursing care, including administering medications, managing ventilators, feeding tubes, tracheostomies, and other medical devices.
Responsibilities:- Conduct Supervisory Visits every two weeks (plus monthly) to our patients at their homes.
- Monitor and assess the child’s condition regularly, recognizing and responding to any changes in status.
- Perform and document clinical assessments, including vital signs, pain management, and overall well-being.
- Implement and follow the child's care plan as prescribed by the physician.
- Coordination of Care:
Collaborate with the healthcare team and other nurses, to ensure comprehensive care. - Communicate any changes in the child’s condition to the physician and healthcare team promptly.
- Maintain accurate and thorough documentation of the child’s care, including progress notes, medication records, and other required documentation.
- Current and unrestricted Georgia Registered Nurse (RN) license
. - Current CPR/BLS certification.
- Valid driver's license.
- Reliable transportation and ability to travel between multiple cities.
- Current automobile insurance.
- Previous home care, home health, private duty nursing, Medicaid, or supervisory experience preferred.
- Strong assessment and documentation skills.
- Excellent time-management and organizational skills.
- Ability to work independently in the field.
- Comfortable communicating with clients, families, caregivers, and agency leadership.
- Experience with electronic documentation/EVV systems is preferred.
