Registered Dietitian
The Pavilion Rehabilitation and Nursing Center is a Five‑Star, Joint Commission‑accredited community offering short‑term, long‑term, and skilled nursing care. Located in Hyannis, we are dedicated to residents and staff and are seeking a fast‑paced, quality‑focused professional.
We are hiring a full‑time, 40‑hour per week Registered Dietitian to join our team.
Responsibilities- Perform medical nutrition assessment.
- Develop nutrition and hydration care plans.
- Conduct ongoing evaluation of all residents.
- Provide consultation and evaluation regarding menu planning, therapeutic diets, food service procedures, personnel, record maintenance, and in‑service programs.
- Complete thorough and accurate initial, quarterly, interim, and annual nutrition assessments for all patients within state and federal guidelines and center policies.
- Complete MDSs and CAAs according to RAI requirements.
- Develop and review individualized nutritional care plans.
- Identify residents at nutritional risk and recommend interventions.
- Collaborate with physicians, nurse practitioners, and team members to create care plans.
- Analyze resident weight data trends and report weight variance concerns to the Director of Nursing.
- Calculate, report, and analyze calorie‑count data as needed.
- Observe compliance with tube feeding (TPN, PPN) documentation and administration.
- Validate intake and output (I/O) data and report concerns to the Director of Nursing.
- Conduct patient and family education on nutrition interventions and therapeutic diets.
- Review meal production and service, monitor menu compliance, portion control, quality, appearance, sanitation, operational efficiency, and temperature adequacy.
- Perform meal‑service rounds and sanitation inspections at least monthly or per the Performance Improvement Program.
- Report results to the Administrator, Director of Nursing, and Dietary Director.
- Registered Dietitian license and active membership in the American Dietetic Association.
- Three years of experience in geriatric, administrative, or clinical nutrition.
- Proficient computer skills.
- Ability to perform duties with or without reasonable accommodations.
