As a Hospice RN Case Manager, you will plan, organize, and direct hospice care as an RN, building from the resources of the community to meet the needs of individuals and families within their homes and communities.
You will provide comprehensive nursing care, supervise aides, and develop or revise plans of care as patient needs change.
