Home Health Case Manager - Registered Nurse

• 12 months of role experience is required with some in the last 12 months.
• Must have at least 12 months of Home Health experience.
• Travel experience is preferred from any number of months.
• 3 months of experience with McKesson is preferred from any number of months.

• Candidates must have a Vermont license or compact license (required after submission).
• Local & travel allowed.
• No current placement allowed at Program:
  Medefis Program - Central Vermont Home Health & Hospice (Non-MSP - VMS).
