We are seeking an experienced Home Health RN Case Manager who is passionate about delivering quality care while enjoying a flexible work environment.

* Key Responsibilities:
** - Provide high-quality patient care in a home health setting - Manage a caseload of patients, ensuring comprehensive assessments and care plans - Utilize OASIS documentation for evaluating patient needs and outcomes - Collaborate with a multidisciplinary team to deliver holistic care - Educate patients and their families on health management strategies - Maintain accurate patient records and documentation

* Qualifications:
** - Minimum of 1 year of nursing experience - OASIS experience essential - Strong background in home health and case management - Preference for candidates with travel nursing experience - Must not have any requested time off during the contract period - Current BLS/BCLS certification required
** Shift Information:** - Tuesday to Saturday schedule - Orientation during the first week (M-F)
