Overview

Registered Nurse – Case Management in Barre, VT, starting 06/01/2026 for a 20‑week travel assignment in a home hospice setting. Responsibilities
  • Provide comprehensive hospice nursing care in patient homes, including symptom management, comfort‑focused interventions, and end‑of‑life care plans.
  • Manage hospice caseloads, assess patient and family needs, develop and revise interdisciplinary care plans, and document progress in the electronic medical record.
  • Conduct regular home visits, monitor pain and symptom control, administer medications as ordered, and coordinate with physicians, social workers, and aides to ensure timely care.
  • Educate patients and families on disease trajectory, goals of care, and medication administration; offer emotional support and caregiver training.
  • Maintain accurate, compliant documentation in the EMR; ensure alignment with hospice regulations and quality standards.
  • Identify and arrange needed resources (equipment, community services, and supports) to optimize quality of life in the home setting; communicate changes to the care team.
Requirements
  • Hospice nursing experience with case management required.
  • Experience providing hospice care in a home setting required.
  • Prior rural travel experience preferred.
  • Experience with McKesson/Netsmart EMR preferred.
  • Availability for a Monday–Friday schedule during the contract period.
  • Active RN license (Vermont) or eligibility for licensure by endorsement; ability to practice in a home‑based hospice environment.
  • Ability to work effectively in a home setting and coordinate with cross‑disciplinary teams.
