Overview

Home healthcare – hospice; travel assignment with rural travel focus
- Provide comprehensive hospice nursing care in patient homes, including symptom management, comfort‑focused interventions, and end‑of‑life care plans.
- Manage hospice caseloads, assess patient and family needs, develop and revise interdisciplinary care plans, and document progress in the electronic medical record.
- Conduct regular home visits, monitor pain and symptom control, administer medications as ordered, and coordinate with physicians, social workers, and aides to ensure timely care.
- Educate patients and families on disease trajectory, goals of care, and medication administration; offer emotional support and caregiver training.
- Maintain accurate, compliant documentation in the EMR; ensure alignment with hospice regulations and quality standards.
- Identify and arrange needed resources (equipment, community services, and supports) to optimize quality of life in the home setting; communicate changes to the care team.
- Hospice nursing experience with case management required.
- Experience providing hospice care in a home setting required.
- Prior rural travel experience preferred.
- Experience with McKesson/Netsmart EMR preferred.
- Availability for a Monday–Friday schedule during the contract period.
- Active RN license (Vermont) or eligibility for licensure by endorsement; ability to practice in a home‑based hospice environment.
- Ability to work effectively in a home setting and coordinate with cross‑disciplinary teams.
- Guaranteed hours per week during the assignment.
- Travel nursing role with a defined contract period.
- Home‑based hospice care focus with rural travel exposure.
- EMR experience with McKesson/Netsmart preferred (prior experience encouraged).
