Registered Nurse (RN) – Home Health

We are seeking an experienced Registered Nurse to provide home health care in Barre and Washington County, Vermont. Flexible schedules, 1:1 care, and a strong focus on work–life balance are integral to this role.

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients within the assigned geographic territory.
• Administer medication, perform wound care, and conduct treatments and procedures as directed.
• Monitor clients’ conditions and report changes to the Clinical or Client Services Manager.
• Execute doctors’ orders and maintain complete documentation.
• Perform client assessments as needed.
• Coordinate case management activities.
• Accurately document observations, interventions, and evaluations using the state‑of‑the‑art touch‑pad tablet.

• Current license as a Registered Nurse in Vermont.
• Minimum of one year of recent clinical experience in a medical/surgical setting.
• Prior home‑care experience preferred but not required.
• Graduation from an accredited nursing program, evidenced by transcript or diploma.
• Ability to work independently, manage time effectively, and travel to assigned cases.
• Strong interpersonal and communication skills.
• Solid computer skills; experience with electronic medical records (EMR) preferred.
