Registered Nurse - Hospice

Job Description

Job Summary:

Provide specialized hospice care to patients in a home setting, ensuring compassionate support and clinical excellence. Manage hospice cases, coordinate patient needs, and deliver customized end-of-life care aligned with best practices.

Responsibilities:

• Assess patients' conditions and develop individualized care plans.
• Deliver high-quality hospice care in patients' homes.
• Manage clinical documentation and update EMR systems accurately.
• Collaborate with interdisciplinary teams to ensure patient comfort.
• Educate families and caregivers on hospice services and symptom management.
• Maintain compliance with hospice regulations and agency protocols.

Qualifications:

• Valid RN license with hospice case management experience.
• Proven ability to deliver hospice care in a home setting.
• Prior experience using McKesson/Netsmart EMR preferred.
• Willingness to travel in rural areas.
• Strong communication and compassionate patient care skills.
• Availability Monday through Friday without RTO requests.
