LPN/LVN - LTC

We are seeking a dedicated LPN/LVN to join a reputable Long-Term Care (LTC) facility. The ideal candidate should have strong clinical skills, excellent bedside manner, and the ability to work as part of a multidisciplinary care team to provide safe, compassionate, and high-quality care to residents.

Key Responsibilities

• Provide direct nursing care to residents in accordance with facility policies and state regulations.
• Administer medications, treatments, and injections as prescribed.
• Monitor patient conditions, record vital signs, and report any changes to the RN or physician.
• Assist with activities of daily living (ADLs) and support resident comfort and mobility.
• Maintain accurate and timely documentation in medical records.
• Collaborate with RNs, CNAs, and other healthcare staff to ensure coordinated care.
• Provide emotional support and build trust with residents and their families.
• Adhere to infection control, safety protocols, and facility guidelines.

Requirements

• Active LPN/LVN license (State of Massachusetts).
• Minimum 1 year of LPN/LVN experience, preferably in Long-Term Care.
• Current BLS certification (AHA preferred).
