American Traveler is seeking an experienced RN for a Home Health position in Vermont, serving as both a direct care nurse and Case Manager on a Monday–Friday schedule with a weekend rotation.
Requirements- Active VT or Compact RN license currently in hand
- Current BLS/BCLS certification required
- Minimum 1 year of RN experience required
- DOB and SSN required for consideration
- Home health setting serving a community-focused patient population in central Vermont
- Schedule is Monday–Friday with a weekend rotation approximately every 4th week
- Day shift position
- Serves as Case Manager when acting as the lead discipline on a case
- Responsible for providing individualized, goal-oriented nursing care using the nursing process in accordance with physician orders
- Provides a range of services including home health, health promotion, long-term care, and maternal-and-child health services
- Organization is a community-focused, not-for-profit dedicated to serving individuals of all backgrounds regardless of financial circumstances
