RN Case Manager – Home Health
Coordinates and delivers skilled nursing care in patients' homes, assesses their medical needs, develops and updates care plans, educates patients and caregivers, monitors progress, and collaborates with physicians and other healthcare providers to ensure safe and effective treatment.
Minimum required qualifications:
- VT/compact RN license
- BLS
- 1+ year recent home health and case management experience
Length of assignment: 16 weeks
Shift / hours per week:
Days, Tuesday - Saturday
