Home Health Nurse

Position:

Registered Nurse (Home Health / Hospice)

Days, Monday-Friday with some weekend coverage

Central Vermont Home Health & Hospice is seeking an experienced Home Health Registered Nurse with strong OASIS and case management experience
. The nurse will provide in-home patient care, focusing on hospice and home health services while managing patient caseloads and coordinating care with physicians and interdisciplinary teams.

Active Registered Nurse (RN) License

Home Health Experience Required

OASIS Experience Required (Manager specifically states "OASIS EXPERIENCE A MUST")

Experience providing patient care in home settings

Ability to work independently in the field

Experience with patient assessments, care planning, and documentation.

Pain and symptom management expertise

Experience coordinating with interdisciplinary care teams

Strong patient and family education skills

Offered pay rate will be based on education, experience, and healthcare credentials.
