Pride Health is seeking a Home Health Registered Nurse for in-home hospice and home health care in Central Vermont. The nurse will manage caseloads, coordinate with physicians, and work with interdisciplinary teams to deliver high-quality patient care.
Ideal candidates have OASIS experience, strong documentation skills, and the ability to work independently in patients’ homes. Experience with end-of-life care is a plus, and some weekend coverage may be required.
