American Traveler is hiring a Home Health RN with required OASIS and home health experience for a Case Manager role requiring a VT or compact RN license.
Details- Home health and hospice care setting serving patients of all backgrounds
- Serves as Case Manager for cases where this role is the lead discipline
- Day shift schedule Monday through Friday
- Weekend rotation required approximately every 4th week
- Homecare Home Base experience preferred
- Active VT or compact RN license required
- BLS certification required
- Minimum of 1 year of experience required
- Home health experience required
- OASIS experience required
- Experience as a home care traveler with OASIS proficiency preferred
- COVID-19 vaccination required with no exemptions
- Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders
- Coordinate care as Case Manager when serving as the lead discipline
