- Responsible for providing individualized goal-oriented nursing care using the nursing process in accordance with a physician's order. In addition, this position serves as Case Manager in cases in which this position is the lead discipline.

• VT/compact RN license
• BLS
• 1+ year home health experience
• Case management experience
• Oasis experience

• Experience as a home care traveler with proficiency in OASIS
• Homecare Home Base experience

13 weeks

Days, Monday-Friday (with weekend rotation approximately every 4th week)
