Hospice Nurse Practitioner

• Conduct face-to-face encounters for patients in their third or subsequent benefit period
• Document and promptly share findings with the certifying physician
• Conduct well-patient checks
• Conduct symptom management visits
• Provide physician support
• Coordinate and collaborate with IDT members, community physicians, facility staff, and others as needed
• Participate in developing Plans of Care for assigned patients
• Enhance care strategies with the Hospice Clinical Manager and Medical Director
• Serve as a resource for Hospice staff to ensure continuity and improve patient care quality
• Attend IDT meetings
• Provide backup support to the Hospice Medical Director and cover medical needs of hospice patients as required

• Graduate of an approved Nurse Practitioner curriculum
• Active APRN or NP license required
• Minimum of two years clinical nursing experience
• Valid driver’s license

Demonstrates expertise in patient care management, including conducting well-patient checks and symptom management visits. Collaborates effectively with interdisciplinary teams to develop and implement comprehensive Plans of Care for hospice patients.
