POSITION SUMMARY
RN Case Manager – Hospice
POSITION DUTIESAssesses patients, develops and coordinates individualized hospice care plans, manages symptoms and pain, provides nursing care and education, collaborates with the interdisciplinary team, and supports patients and families to ensure comfort and quality of life during end-of-life care in the home setting.
MINIMUM REQUIRED QUALIFICATIONS –- VT/compact RN license
- BLS
- 1+ year home health hospice case management experience
- Oasis experience
16 weeks
SHIFT / HOURS PER WEEKDays, M-F with some weekend coverage
SYSTEMSOasis, Homecare Homebase HCHB
