Job Summary
We are seeking a compassionate and experienced Social Worker to join our team at Berlin Health & Rehabilitation Center in Barre, Vermont. As a Social Worker, you will play a critical role in providing medically‑related social services to our residents, ensuring that their physical, emotional, and social needs are met. This is a part‑time or full‑time opportunity, depending on the needs of the facility.
Responsibilities- Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department to ensure that residents receive high‑quality care and attention.
- Identify the need for medically‑related social services and ensure that these services are provided in accordance with State and Federal regulations.
- Complete the social services component of the comprehensive assessment for assigned residents and contribute to and/or direct social services goals and approaches to the comprehensive care plan.
- Facilitate residents' safe transition back into the community through interdisciplinary discharge planning and arrangement of community‑based services and follow‑up care.
- Assist residents and their representatives in locating and accessing financial, legal, and other community resources.
- Interpret and report social, psychological, and emotional needs of residents/responsible parties to the medical staff, attending physician, and other resident care team members.
- Accurately and completely document social service actions and interactions in each resident's medical record.
- Engage in Advance Care Planning for assigned residents and ensure that Advance Directives are reviewed with the resident/resident representative on a regular basis.
- Assist with facility policy development and annual review to positively impact the quality of care delivered to residents.
- Advocate for residents and assist them in asserting their rights within the facility.
- Report suspected abuse, neglect, or exploitation to the Social Services Director and/or Administrator and assist with reporting to the appropriate State agency.
- Ensure that residents who display mental illness or psychosocial difficulties have access to appropriate treatment and resources.
- Bachelor's degree in social work or another human services field, including sociology, gerontology, special education, rehabilitation counseling, or psychology.
- One year of supervised social work experience in a healthcare setting working directly with individuals.
- Strong communication and interpersonal skills.
- Ability to work in a fast‑paced environment and prioritize multiple tasks and responsibilities.
- Knowledge of State and Federal regulations related to social work in healthcare settings.
- Ability to maintain confidentiality and handle sensitive information.
- Strong problem‑solving and critical‑thinking skills.
- This position will be based at Berlin Health & Rehabilitation Center in Barre, Vermont.
- The Social Worker will work closely with the Social Services Director, medical staff, and other resident care team members to provide high‑quality care and attention to our residents.
